RESPONSE to Safety Recommendation:

The second response of [Hellenic] Civil Aviation Authority (HCAA) to AAIASB Safety Recommendation 2022-08 was received by email on 21/3/2023.

[Second HCAA Response]:

“ HCAA's internal procedures have been reviewed and revised, and the inspection plan for the year 2022-2023 includes inspections in all areas of control of organizations in accordance with the European regulation 965/2012 with new EASA inspection Check-Lists that are available to inspectors of HCAA from 01/01/2023. The specific new HCAA procedure fully ensures compliance with the approved procedures for the inspection of the Organizations. ”

[Hellenic National Agency for the Investigation of Air and Rail Accidents & Transport Safety (former AAIASB) assessed this response as “Adequate” on 03/04/2023 and changed the SR 2022-08 status to “Closed”]



History of previous response:

The [Hellenic] Civil Aviation Authority’s (HCAA) first response to AAIASB Safety Recommendation 2022-08 was received by email on 31/1/2023.

[HCAA’s first response]:

“ We would like to inform you that following the issuance of the Final Report [AAIASB Final Report 04/2022] and the safety recommendations of the Investigation Board [AAIASB] to the HCAA regarding the accident that occurred at the Poros-Galata Sea Channel on 20/8/2019, the Flight Operations Department of the Directorate for Flight Standards issued the following recommendations addressed to all air carriers. (Safety Recommendation 2022-07, 08)

Flight Safety Recommendation 2022-08

The [Hellenic] Civil Aviation Authority recommends that the Company/ies [operator(s)] include in every flight containing a landing/takeoff in a field not approved by the Authority, a risk analysis assessment prior to the commencement of the route (Risk Assessment) which will be a necessary accompanying document.

The [Hellenic] Civil Aviation Authority advises helicopter operators to keep PART C [Operations Manual, Part C] up to date with all possible aerodromes and landing fields. Alternatively keep a Directory of Helipads up to date with detailed information and instructions for the approach, landing and take-off taking into account all the possible difficulties that the crew may encounter.

In addition, companies are recommended in cases where a route and Take-off or Landing is required in fields that have been identified as areas of great difficulty, to use only crews that have recent experience on the specific route and designated as experienced by the air carrier. (The recommendation is sent to the helicopter operators).”

[Hellenic National Agency for the Investigation of Air and Rail Accidents & Transport Safety (former AAIASB) with its document 349/14.03.2023 assessed initially within the 60-day deadline the addressee’s initial response as “Partially adequate” and changed the status of Recommendation 2022-08 to “Open — Partly adequate” stating that the corrective actions described in the addressee’s response, although correct, do not refer to the actions that the HCAA should take to ensure compliance with the approved inspection procedures to the Organizations, asking for these actions to be completed.]

From the wreckage examination, failure was identified of Part 145 Maintenance Personnel to comply, in the context of scheduled Maintenance, with the Maintenance task card requirements regarding the identification of No. 1 Engine faulty parts (worn breather flexible tube and damaged starter generator Cable).

It is recommended to the Maintenance Organization to make sure that all procedures included in the Maintenance Organization Exposition (MOE), are followed.

RESPONSE to Safety Recommendation:

Response to AAIASB SR 2022-06 was received by email on 08/9/2022.

[Response]:

“ a. Technical info notice (TIN) 09_2022 dated 23/6/2022 has been issued to inform all the technical staff of the Organization, regarding the findings recorded and where of the Maintenance Organization concern .

b. Internal document 22/056 dated 27/6/2022 has been issued to define additional procedures, supplemental to those already mentioned in the Maintenance Organization Manual (MOE) and should be followed from the technical staff.

c. At the same time, the TIN 2a_2021 version regarding the Alert Commercial Service Letter (ACSL) of Rolls-Royce, (Maintenance Warning/External Lines), dated 26/7/2021, is highlighted to inform all the organization technical staff. The said ACSL pointed out the need to faithfully apply the procedures mentioned in the technical manuals (Operation and Maintenance Manual).

d. In addition, the existence of two inspections/signatures for all the tasks performed by the maintenance organization (performed by / inspected by) is emphasized, before the release of the aircraft (CRS issuance). ”

Recommendation PREAMBLE:

From the review of the Captain’s Logbook during the investigation, it was identified that the Captain’s total Helicopter flight hours were performed in a Multi Crew Environment. In this context, the transition of the Captain to a single-Pilot flight environment, resulted in the increased workload, with a possible negative effect on the situational awareness and at the same time risk increasement.

The second response of [Hellenic] Civil Aviation Authority (HCAA) to AAIASB Safety Recommendation 2022-07 was received by email on 21/3/2023.

[Second HCAA Response]:

“ The recommendation for the "vice versa" case does not apply in the Greek territory as there is no organization authorized to operate Helicopters in a dual crew environment.

In Greece all helicopter airlines with AOC as well as private helicopters operate only as SINGLE PILOT helicopters.

In addition, the European regulation 965/2012 (b related) [EU Regulation] provides detailed special qualifications, extensive special training and a special training for the promotion of a single-pilot helicopter commander to a dual-pilot helicopter commander, which apparently covers, if required in the future, the requirements of the specific recommendation. ”

[Second HCAA Response]:

“ The internal procedures of the HCAA have been revised and since 01/01/2023 EASA’s updated Check-Lists have been adopted, which are in accordance with European Regulation 965/2012 as referred to in (b) above [REGULATION].

Since then, all our inspectors have used the specific Check-Lists to compare the manuals when checking the OM-A, B, C, D, [Operation Manuals] of the organizations thus ensuring that there are no contradictions. ”
